Community Access by Peace United Care offers numerous advantages to individuals with disabilities who wish to actively engage in their local community. Our Carers are dedicated to dismantling barriers and facilitating meaningful engagement, empowerment, and inclusion.

Our Clinical Manager will accept assessments to provide to our highly experience Registered Nurses to conduct the following Assessments:
- Health Assessments
- Continence Assessments
- Bowel Assessments
- Skin Assessments
- Overall Health and Wellbeing Assessments
- Wound care Assessments
- Abbey Pain Scale
- Hospital Anxiety and Depression Scale (HADS)
- All relevant Care plans for Continence, Bowel, Medication etc
- The Barthel Index Assessment
